AI Project Risk Review

A considered reading on where your AI project really stands.

This tool delivers a structured review of an AI project in build, being bought or currently running. It consists of about 40 multiple-choice questions across 5 domains: Decision profile, People, Data & technology, Governance and Regulatory – written to be answered by anyone close to the project.

The output is a detailed report on the level of risk that you may be exposed to, together with recommendations on next steps.

What this is

A diagnostic tool for project leaders who want a considered second view, without committing to a conversation. The report is yours to share or keep.

What this is not

A compliance audit, a scorecard, or a substitute for a DPIA. We are aiming for useful clarity in about twenty-five minutes, not a regulatory verdict.

What we do with your answers

The tool runs entirely in your browser. We do not log your answers or ask for an email. Closing the tab clears the session. Two anonymous counts are kept — that a review was started, and that a report was generated — and nothing else.

Frameworks & scope

Informed by established AI risk frameworks, including the NIST AI Risk Management Framework and the EU AI Act. It maps to their principles to structure your thinking; it is not a compliance check against any of them.

Frequently asked questions

What is it?

The role of this tool is to provide a view on an individual AI capability that identifies any areas of concern or risk. It covers five domains: Decision profile, People, Data & technology, Governance and Regulatory.

Who is it for?

Any business or technology leader who is involved in an AI project and would like an independent view on whether there is anything to be concerned about.

How long does it take?

It should take around 25 minutes. The questions are set at a level where anyone with a working knowledge of the project can answer the majority without having to research — no detailed technical knowledge is needed.

What will I get?

A sample report can be found here, based on a hypothetical AI project with multiple concerns to show the breadth of the report.

What does it cost?

Zero.

How is my data used?

The tool runs completely in your browser, and the only data collected is a small counter to show us how many times the tool has been used and how many reports generated. This is just a counter — no information about the user, or the answers given, leaves your browser (unless you choose to download it).

Do I have to answer all the questions in one pass?

No. You can leave the questionnaire open in your browser and come back at any time, though your answers will be lost if you close the tab or the browser. You can also navigate backwards and forwards through the questions as you wish; there are no gates.

What can I keep?

You can download the report produced, and the full set of questions with the answers that you gave.

Can I change my answers?

Yes. As long as the browser session remains open, you can go back and review or change your answers, before or after you have generated a report.

Does this replace the need for audit?

No — it is a tool to give feedback and guidance based on the answers given. It does not check the validity of the answers, or go into the depth needed for an audit.

Is this check up to date with current regulation?

Yes, it is aligned to NIST and EU guidance. The tool was last updated on 8 June 2026.

Thanks for completing the questionnaire.

You can now generate the report and download a copy of the answers you gave. The report will show the findings across the 5 themes, together with a summary and a list of any high-priority recommendations.

The findings are categorised as Urgent, Important and Worth noting, based on the assessed criticality.

At the end of the report document, there are options to:

  • Download as a PDF
  • Download the answers you gave
  • Go back and edit your answers
  • Restart from scratch.
